Program Overview

2nd Home Day Hab Center offers a comprehensive day habilitation program for adults with intellectual and developmental disabilities, serving a broad range of ability levels in a safe, nurturing, and supportive environment.

Our program is designed for participants aged 21 and older who benefit from structured activities, skill-building, and social engagement throughout the day.

Our compassionate Direct Support Professionals are creative, encouraging, and dedicated to helping each individual reach their highest level of independence.

Transportation & Accessibility

We provide safe door-to-door transportation for all participants, ensuring easy, reliable,
and accessible travel to and from the center each day.

Our vehicles are maintained to the highest standards, and our drivers are trained to provide
courteous, supportive service to every individual.

Medicaid & Enrollment

The 2nd Home Day Habilitation Program is approved for Medicaid participants, and our team
will gladly assist families with questions regarding eligibility, enrollment, or required documentation.

We also collaborate closely with families, support coordinators, and care professionals
to create a fulfilling and enriching experience tailored to each participant’s goals.
